Home health care is one of the most rewarding — and most vulnerable — positions in healthcare. You enter strangers' homes, often alone, in neighborhoods you don't know, with limited backup if something goes wrong.
Your agency probably gave you some training. It probably wasn't enough. This guide fills the gaps.
Before the Visit
Research the address. Google Street View takes 30 seconds. Know what the neighborhood looks like, where you'll park, and if there are any obvious concerns.
Review the patient history. Any behavioral issues? Anyone else in the home? Pets? The more you know going in, the better.
Tell someone your schedule. Your supervisor, a colleague, a family member. "I'll be at [address] from 2-3pm. Text me if you don't hear from me by 3:15."
You have the right to leave.
Always.
At the Door
Pause before knocking. Look at the environment. Is there anything that makes you uncomfortable? Aggressive dogs? People loitering? Trust your instincts.
Listen. Before entering, listen at the door. Yelling? Crying? Signs of a crisis you're about to walk into?
Position yourself. When the door opens, stand to the side, not directly in front. This gives you a better angle to see inside and react if needed.
Inside the Home
Scan the environment. Note the exits. Note who's present. Note anything that could be a weapon or a hazard.
Keep your exit clear. Don't let anyone get between you and the door. If someone moves that direction, you move too.
Trust discomfort. If something feels wrong — even if you can't articulate why — listen to that feeling. You can always reschedule.
"I need to get something from my car." "I'm getting a call I need to take." "I forgot to confirm something with the office." You don't need to explain. Just leave.
After the Visit
Document everything. Not just the clinical notes — anything safety-related. This protects you and helps the next person who visits.
Check in. Let your designated contact know you're done and safe.
Debrief. If anything felt off, talk to your supervisor. Your instincts might protect a colleague later.
